$15 for either Early or After Care, $30 for Both.
*Note that payments are made for each day and do not cover for all 3 days*

Instructions to Register
1. Fill in all of the required (*) boxes
2. Read our policies, terms, and conditions. Accept the policies, terms, & conditions by checking the box below for the submit button to appear. Then click submit.
3. Upon submitting the form, we will reply via email with the final amount owed based off of how much days and type of care that is needed.
4. Interac E-transfer: chessmindscalgary@gmail.com the stated 'Before/After care' fee in order to fully register for our Before/After Care.

Our Policies, Terms, & Conditions
Late Parent/Guardian Pick-Ups
Chess Minds Calgary is not liable to look after students/children past 4:40 pm. The before/after care payments ensure that the participants are cared for during the specified times which were paid for. Please ensure that the designated parents/guardians are able to pick-up the participant on time. If a parent/guardian (who has not paid for after care) does not pick up their child between 4:15pm-4:30pm, they will be assessed a $25 penalty fee with an additional $25 for every half-hour after that. (payment must be sent before the next day in order for the participant to join the remaining camp days)
Liability
Cancellation & Refund Policy
Chess Minds Calgary is not liable to take care for students/children of those who have not paid for our Before/After/Both care

Enrollment can only be cancelled within the first 24 hours from the time the payment was received: after which, the payment is considered final and non-refundable.

Exceptions: In the event that the student needs to withdraw from the program due to medical reasons, a valid physician's note is required. An administrative fee of $20 will be charged. Refund will be subject to pro-rated computation based on the remaining sessions.

Missed Days: For any missed days, Chess Minds Calgary will not provide any refunds and make up days.

Personal Info/ Photo Release
Personal information provided upon registration will be collected and protected under the Freedom of Information and Protection of Privacy Act. Collection is used to maintain records for enrollment in the Chess Minds Calgary and corresponding with registrants and their parents. Personal Information may be used or disclosed for other operational purposes that are consistent with the requirements of federal or provincial legislation. The participant’s name and photograph may appear in the Chess Minds Calgary website http://www.chessmindscalgary.ca and and all its social media web pages.
Medical Treatment Permission
If a student requires immediate medical attention, the parent/guardian listed on the emergency contact list will be contacted. If unable to reach the parent/guardian, the student will be transported to the nearest emergency establishment.

Medical expenses incurred will be shouldered entirely by the parent/guardian.
Claim Release
Chess Minds Calgary is not liable for any personal injuries, property damage, or lost/stolen items while participating in chess activity, before or after-class, or while on the premises.
